Reliant Hospice, Inc. provides Californians with access to essential services for end-of-life care. As a provider of hospice care in Montclair, California, we help terminally ill patients and their families find comfort and security in a time of great distress.

We gladly serve patients in their own homes or in Long-Term Care Facilities, Nursing Homes, Assisted Living Facilities, and Residential Care Facilities for the Elderly. What Is Hospice?

Hospice is a specialized way of caring for people who have limited life expectancy. Reliant Hospice, Inc. works with the patient’s physician using a palliative care approach. This renders physical, emotional, and spiritual support to patients, families, and their loved ones. We focus on enhancing the quality of life rather than the length of life. Our hospice services are available 24 hours a day, 7 days a week.

Who Can Avail of Hospice Care?

Hospice care is a special benefit under Medicare (hospital insurance). You qualify for Medicare hospice benefits when:

  • You are eligible for Medicare Part A;
  • Your doctor and the hospice medical director certify that you are terminally ill and have less than six months to live;
  • You sign a statement choosing hospice care instead of routine Medicare-covered benefits; and
  • You received care from a Medicare-approved hospice program.

What Is Our Mission?

Our mission is to provide an individualized program of physical, emotional, and spiritual care for people in the last phases of a life-limiting illness, focused on managing pain and other symptoms. Our program is reflective of the spirit of caring that emphasizes comfort and dignity for the patient, making it possible for them to remain independent for as long as possible, in familiar surroundings.

What Does Hospice Insurance Cover?

  • Hospice care is fully covered by Medicare and Medi-Cal benefits.
  • Most private insurance and managed care plans have hospice benefits.
  • Care provided by the attending physician and treatment for conditions not related to the life-limiting illness continues to be paid by Medicare and Medi-Cal.
  • The patient or their representative can revoke hospice to return to a curative treatment plan at any time. The patient may also re-elect hospice care and move into the next period when the need arises.
